Important Information For Community First Medicare Advantage Dual Eligible Members

Oct 14, 2026 | Member News, Member Resource

On January 1, 2027, there will be important changes to the way Bexar County residents will receive their Medicare and STAR+PLUS (Medicaid) services.

If you live in Bexar County and are currently enrolled in Community First Medicare Advantage Dual Eligible Special Needs Plan (HMO D-SNP) for your Medicare services and also enrolled in a STAR+PLUS (Medicaid) plan either through our health care company or another health care company, on January 1, your Community First Medicare Advantage Dual Eligible Special Needs Plan will end. You will be enrolled in a new health plan, Community First Medicare Advantage Bexar Dual (HMO D-SNP). This new Fully Integrated Dual Eligible Special Needs health plan:

  • Contracts with both Medicare and STAR+PLUS (Medicaid) to provide benefits of both programs to Members.
  • Was created to simplify your Medicare and Medicaid coverage and improve coordination of your care.
  • Is provided by Community First Health Plans, which is the same company that currently provides your Medicare services through Community First Medicare Advantage Dual Eligible Special Needs Plan (HMO D-SNP).

Unless you make a different choice, we will automatically enroll you in Community First Medicare Advantage Bexar Dual on January 1. This means you will not be enrolled in Community First Medicare Advantage Dual Eligible Special Needs Plan and your separate STAR+PLUS (Medicaid) plan starting January 1.

Transition Details

  • You will continue to get Medicare services and STAR+PLUS (Medicaid) services through your current health plans through December 31, 2026.
  • On January 1, you will start getting your Medicare services and STAR+PLUS (Medicaid) services through Community First Medicare Advantage Bexar Dual.
  • You do not need to do anything to enroll.

Review New Plan Information

Carefully read the Annual Notice of Change and Member Handbook (Evidence of Coverage) to determine if Community First Medicare Advantage Bexar Dual (HMO D-SNP) will meet your health care needs, including those for long-term services and supports.

You can choose not to automatically enroll in Community First Medicare Advantage Bexar Dual. Participation in our plan is optional. Your Medicaid eligibility will not be affected by your Medicare choice.

Members who live in Atascosa, Bandera, Comal, Guadalupe, Kendall, Medina, or Wilson county and are currently enrolled in Community First Medicare Advantage Dual Eligible Special Needs Plan will be transferred to Community First Medicare Advantage Mezquite Dual (HMO D-SNP).
