Medicare Advantage Alamo Plan (HMO) is an all-in-one plan designed to offer everything you need to live your healthiest life, including:
- Hospital coverage (Medicare Part A)
- Medical coverage (Medicare Part B)
- Prescription drug coverage (Medicare Part D)
Plus additional benefits you won’t get with Original Medicare, like coverage for dental, vision, hearing, fitness, and more.
Medicare Advantage Dual Eligible Special Needs (D-SNP) Plan
Community First also offers a Dual-Eligible Special Needs Plan (D-SNP) designed for people diagnosed with certain conditions or diseases, or those with low incomes.
As a Medicare Advantage D-SNP (HMO) Member, you will receive all the benefits included with the Medicare Advantage Alamo Plan (HMO) in addition to services to help manage your health care needs, including:
- Interdisciplinary Care Team
- Care Coordinator
- Individualized Care Plan
Community First Member Portal
The Member Portal acts as your own secure website where you can log in with your unique username and password and quickly access information about your health plan. By using the Member Portal, you can save time and take better control of your health care coverage. In just a few clicks (instead of a phone call) you can change your primary care provider, view or request a new Member ID card, check your eligibility, update your personal information, and more.
You can also access the Member Portal from any device, and toggle to view it in your preferred language!
