Community First offers a Dual-Eligible Special Needs Plan (D-SNP)
You can get a Dual Eligible Special Needs Plan (D-SNP) through Community First Health Plans after you’ve enrolled in Original Medicare Part A and Part B.
Our Community First Dual-Eligible Special Needs Plan (D-SNP) is a Medicare Advantage HMO Plan, so it covers Medicare Part A and Part B. It also includes coverage for prescription drugs, dental, vision, and hearing.
Designed With Your Health In Mind
Community First Medicare Advantage Dual Eligible Special Needs Plan (HMO D-SNP) is designed for people who have Medicare and who are also entitled to assistance from Medicaid. Since you get assistance from Medicaid with your Medicare Part A and B cost sharing (deductibles, copayments, and coinsurance), you may pay nothing for your Medicare health care services.
Extra Help When You Need It
Medicaid may also provide other benefits to you by covering health care services such as prescription drugs, long term care, and/or home- and community-based services that are not usually covered under Medicare. You will also receive “Extra Help” from Medicare to pay for the costs of your Medicare prescription drugs. Community First Medicare Advantage Dual Eligible Special Needs Plan (HMO D-SNP) will help manage all of these benefits for you, so that you get the health care services and payment assistance that you are entitled to.
