Community First Medicare Advantage Bexar Dual Plan:
Coordinated Coverage for People with Medicare and Medicaid
Important changes are coming for Bexar County residents who receive both Medicare and Medicaid (STAR+PLUS). Community First Medicare Advantage Bexar Dual Plan (HMO D-SNP) is a new plan available to Bexar County residents, designed to align your benefits for a better health care experience.
What is Changing?
Texas is expanding options to help people who have both Medicaid (STAR+PLUS) and Medicare receive more coordinated care. Beginning January 1, 2027, your STAR+PLUS and Medicare coverage will be aligned through a new integrated Medicare plan. This means your STAR+PLUS health plan and your Medicare Advantage Dual-Eligible Special Needs Plan (D-SNP) MUST be managed by the same organization.
What This Means for You
If you live in Bexar County and have Medicare and STAR+PLUS through different health plan carriers, your coverage will be aligned under your Medicare health plan in 2027.
The Medicare plan you choose will provide both your Medicare and Medicaid benefits. The plan you choose makes all the difference.
Community First Medicare Advantage Bexar Dual is a new plan designed to integrate your benefits, providing over 30 years of trusted support and quality coverage from Bexar County’s only local, non-profit health plan.
Guidance for Community First STAR+PLUS Dual Members
If you are currently a Community First STAR+PLUS Dual Member and have Medicare coverage from another health plan (not Community First), you have the option to switch plans to Community First Medicare Advantage Bexar Dual during the annual open enrollment period beginning October 15.

If you do not enroll in Community First Medicare Advantage Bexar Dual the state may assign your STAR+PLUS coverage to a different health plan organization so that your Medicare and Medicaid benefits align.
Participation in our integrated Medicare Advantage Bexar Dual plan is optional. Your Medicaid eligibility will not be affected by your Medicare choice.
