About Community First Integrated D-SNP
Community First Medicare Advantage Bexar Dual (HMO D-SNP) is a Fully Integrated Dual Eligible Special Needs Plan option for residents of Bexar County who have both STAR+PLUS and Medicare.
- Bexar Dual will provide improved care coordination, streamlined authorizations, and reduced administrative burden.
- Bexar Dual integrates Medicare and Medicaid benefits, including Long-Term Services and Supports (LTSS), behavioral health, and preventive care.
- Providers will only need to submit one claim through Community First for Medicare and Medicaid services.
- Bexar Dual Members will receive one ID card, one care team, and one Provider network.
Providers should always verify eligibility and coverage prior to rendering services.
To continue serving existing Community First dual eligible patients, Community First Providers may need to contract with Community First D-SNP and/or STAR+PLUS Medicaid. Call Provider Services at 210-358-6030 or email NManagement@cfhp.com for questions about your provider contract or fill out our Letter of Interest.
As a reminder, dual-eligible Members may be more vulnerable and have more complex care needs than Members who are enrolled in only Medicare or Medicaid. Awareness of which factors apply to your dual-eligible patients is critical to understanding their care needs.
