Please complete the survey linked below no later than February 6, 2024, to ensure your practice receives vital HEDIS® notifications in a timely manner.
MEDICAL RECORD RETRIEVAL SURVEY
The survey includes questions about the appropriate point of contact and your practice’s medical recordkeeping process. Your responses will ensure efficient and accurate collection of medical records.
Community First recommends using one or a combination of these methods to complete medical record retrieval requests:
- EMR remote retrieval
- ShareFile (Community First’s secure portal upload)
- Fax
